The following attachments are attached to this notice: Attachment 1 ? Schedule and Contract Clauses Attachment 2 ? Price Proposal Breakdown Attachment 3 ? Representations and Certifications Attachment 4 ? IRS Form W-14 Attachment 5 ? Drawings and Photos will be made available separately through FAN.gov.
